All Required Forms Below Should Be Completed, Signed and Emailed to Jen Engle, DP Middle School Athletics Secretary.

1. Enrollment Form - Complete Form Online.

3. WIAA Clearance Form- Complete and Email Sports Physical Form

All students planning to participate in a middle sport during the 2025-2026 school year must have a physical on file at the school, dated after April 1, 2025, prior to starting official practices.

Athletic Fees

$21/year, regardless of the number of sports. Pay online through the parent portal.   Go to Fees->My Account (upper right hand corner)->Optional Payments->MS Athletics. 

6th graders and any approved 5th graders will need to make payment directly via a check.  Please bring to first practice.
